How we may use and disclose health information

We may use and share your health information to provide, coordinate, or manage your dental care. This may include sharing information with dentists, physicians, laboratories, pharmacies, specialists, or other healthcare professionals involved in your treatment.
Payment
We may use and disclose health information to bill and collect payment for services, determine benefits or coverage, obtain prior approval, and coordinate with dental or medical benefit plans.
Healthcare operations
We may use and share information for activities needed to operate the practice, such as quality assessment, staff training, credentialing, compliance, auditing, business planning, and improving patient care.
People involved in your care
When appropriate, we may share information with a family member, friend, personal representative, or other person involved in your care or payment for your care. We may also share limited information to assist with disaster-relief efforts or notify someone about your location or condition.

- Inspect or obtain a paper or electronic copy of health information maintained in your designated record set.
- Ask us to correct or amend information you believe is inaccurate or incomplete.
- Request an accounting of certain disclosures made during the period allowed by law.
- Ask us to restrict particular uses or disclosures. We are not always required to agree, except in circumstances required by law.
- Ask us to communicate with you by a reasonable alternative method or at a different location.
- Receive a paper copy of this notice, even if you agreed to receive it electronically.
- Choose someone with legal authority to act on your behalf.
